Mrs Nash and Mrs Richards are based in the school office looking after the needs of the children, teachers and to help in the smooth running of St.Giles’. They are happy to help with any questions you may have and will be the first point of contact for all administration queries. However, for your convenience, you can download and fill in a number of forms below, which can be dropped off into the School Office once completed. Hard copies of all documentation are available from the School Office on request.
NOTIFICATION OF ABSENCE FORM
If you need to take your child out of school during term time this form needs to be completed and then submitted to the headteacher.
ADMINISTRATION OF MEDICINES
Complete this form if your child needs medication during the school day.
SPECIAL DIET REQUEST FORMS
Complete this form if your child requires a special diet or suffers from any allergy.
FREE SCHOOL MEALS
In addition to the Universal Free School Meal (Years R-2) from the Government, your child may be entitled to get help with other school costs, if you are on a low income and get benefits. By completing this short form we can quickly and easily check whether your child is eligible.
SWIMMING CONSENT FORM
During the academic year every child will have the opportunity to have swimming lessons at the Freemen’s swimming pool. This consent form needs completing prior to your child swimming.
COOL MILK
Once your child reaches the age of 5 years they are no longer eligible for free milk in school. If you would like them to continue to have milk at break time you can contact Cool Milk direct, complete the form and arrange payments.
CHANGE OF EMERGENCY CONTACT DETAILS
At the beginning of each academic year you will be asked to complete an emergency contact form. If your details change during the year please submit a new, updated form.
